Preventing chronic illness through software
Counting over 4.5 million policyholders, AOK Baden-Württemberg is one of the biggest health insurance companies in the South-West of Germany.
They provide full health protection across 230 physical branches and several online services, and they are also heavily involved in illness-prevention.
AOK supports its clients in preventing many chronic diseases by encouraging healthy habits and helping them maintain a balanced lifestyle.. It is in the area of prevention that Cognostics intervened during this project, supporting AOK in finding a way to ensure that everyone can be supported in implementing the difficult changes they face when fighting the onset of chronic disease.

The Challenge
It’s hard to cure who you can’t reach
In 2019, 86% of all deaths within the EU were attributable to non-communicable chronic diseases.
These are widespread ailments such as diabetes, cardio-vascular disease, cancer and chronic respiratory diseases.
However, while it is known that lifestyle changes could prevent or delay their onset significantly, it is also true that conventional prevention efforts have so far failed to reach some of the most at-risk populations.
What was the best way to solve this issue?
The Solution
Inducing change, not enforcing it
We brought together a team of psychologists, designers, AI specialists and developers to create a sophisticated psychological model.
Rather than assuming a positive attitude towards lifestyle transformation, the model eases patients into it.
We then placed the model into a companion mobile application that challenges the users, increasing their awareness of their own harmful habits.
Our solution brings together interdisciplinary knowledge to overcome the biggest obstacle patients have when changing their lifestyle: themselves.
On top of this application, we are developing a second one that targets healthcare professionals and their needs.
